A PURCHASE WILL NOT IMPROVE YOUR CHANCES OF WINNING. “Vitamin Sweepstakes of a Lifetime” begins 06/02/14 and ends 08/31/2014. Winners of grand prize and weekly prizes below will be selected from among all eligible entries received in a random sweepstakes conducted on or about September 15, 2014 by Indiana Botanic Gardens, Inc., whose decisions are final. Winners will be notified within 2 weeks of selection. All prizes will be awarded. Open to legal U.S. residents and U.S. territories and possessions, 18 years or older and only where offered & permitted by law. Void where prohibited by law. Odds of winning depend on the number of eligible entries received. No person will be eligible to win more than once. For a list of winners, available after 10/31/2014, send a self-addressed, stamped envelope by 09/15/2014 to: “Vitamin Sweepstakes of a Lifetime” Winners, Botanic Choice, 3401 West 37th Ave., Hobart, IN 46342. Winner’s list will be available for 60 days after winner selection. Please allow 6 weeks for delivery.
Vitamin Sweepstakes of a Lifetime 2014 Consumer Disclosures
You have not yet won. No purchase necessary. One grand prize winner will receive a $500 gift certificate annually for three consecutive years, and twelve weekly winners will each receive a $50 gift certificate each good towards Botanic Choice merchandise. The weekly winners will be notified the week following the drawing. The grand prize will be notified by 09/22/2014. Both the weekly winners and the grand prize winner will be notified via phone and also announced in an email follow up. Name initials and state may appear on botanicchoice.com at the discretion of Indiana Botanic Gardens, Inc. Sweepstakes ends 08/31/2014. Winners will be chosen on or about 09/15/14. If winner(s) is/are ineligible, unavailable or unwilling to accept the prize of the gift certificate within 15 days of notification, an alternate winner will be selected at random from remaining entries received. No cash substitution.
How to Enter
Enter online at https://www.BotanicChoice.com/lifetime. Online entries must be received before 11:59PM CST, Monday, September 1, 2014. Enter via mail in lieu of online by hand printing your name, address, daytime telephone number, & email address on a 3 x 5 card & mailing it to: Vitamins Sweepstakes of a Lifetime, 3401 West 37th Ave., Hobart, IN 46342. Mailed entry must be postmarked by August 31, 2014. Only one entry per person, per email and/or mailing address is permitted, regardless of method of entry.
